英国大学学位保留申请书如何写

英国大学学位保留申请书如何写 是想延期offer还是在读准备休学？
第一种情况请问清楚是否可以延期，下面的邮件发给学院招生秘书，2天没反应打电话过去。下面的邮件是已经用过，确实可行的说法。
Subject: Offer defer application, ID: 你的学号
To whom it may concern
This is from an offer holder, ID: 你的申请号/学号
I was wondering if I could defer my offer from 2014 to 2015, September (如果确实想15年9月入学的话), due to 你的原因.

第二种情况建议直接去学院办公室，如果想发邮件的话可以如下格式:
Subject: Convalescent Leave application, ID: 你的学号 (这是生病休学申请，如其他原因请按实际情况修改)
To whom it may concern
This is from a Master/PhD/Bachelor student, ID: 你的学号
I was wondering if I could take a leave from 2014 to 2015, September (请按实际日期填写), due to 你的原因. Would you kindly let me know the necessary procedures and documents to prepare?
